TORONTO - A north-end Toronto supermarket has been fined $90,000 because of the death of a worker on Dec. 27, 2007.
The worker was using a lifting device at Tuong Phat Supermarket to move a shopping cart to a basement storage area.
The device's forks tipped backwards, causing the machine to fall, fatally crushing the worker.
An investigation found that there were no written instructions for the safe operation of the lifting device.
The supermarket pleaded guilty to failing to ensure the worker was given instructions regarding the safe use of a lift truck.
Tuong Phat Supermarket was fined last Thursday under the Occupational Health and Safety Act and the information was released today.
