Ontario's Special Investigations Unit has identified the 49-year-old man who died after police opened fire at a home in Markham, north of Toronto.

The arms-length agency which investigates incidents where there has been a death or serious injury involving police, said Kaveh Sabouri Tabatabeai sustained a fatal gunshot wound late Monday after two York Regional Police officers opened fired.

The agency said police responded to a call on Cottonwood Court at approximately 11 p.m

CTV's Natalie Johnson reports that police found a knife-wielding man at the scene, who began stabbing himself. After the man allegedly lunged at police, two officers opened fire and Tabatabaei was shot.

York Regional Police said officers discovered a 47-year-old woman suffering from apparent stab wounds.

Both Tabatabaei and Nasrin Toreihi died at the scene.

Neighbours have described the man and woman as a married couple who lived in the home with their two daughters.

Neighbour Tony Dong said it was not the first time police were called to the home for a domestic disturbance.

"That couple had trouble for some time and . . . the police came and the ambulance too, some time ago," he told CTV Toronto.

Police went door-to-door along the street Tuesday, asking neighbours what they heard Monday night or what they knew about the couple.

The SIU is probing the actions of police, while York Regional Police are investigating the woman's death.

This is the second incident involving SIU over the weekend.

On Saturday, a 30-year-old Ajax man was shot by Toronto Police officers after the conclusion of the Caribbean Carnival parade in downtown Toronto. Two other people were also injured during the incident.
